Rates and Insurance

Rates

$130 for a standard session (sliding scale is available).

If using insurance, rates will vary depending on the insurance provider. I am in network with BCBS, Anthem, Carelon, Quest, Cigna and Aetna. To verify insurance and check benefits go to Headway.co.

Credit cards (Mastercard, Visa, American Express and Health Savings) are accepted for payment. 

Cancellation Policy: 24-hour notice is required if you cannot attend a scheduled appointment.  No-shows and cancellations without a 24-hour notice will be charged $50.00 (unless discussed with Dale and waived).  This fee cannot be claimed on insurance.
